Payroll Forms:
To change your payroll tax withholding status or other deduction withholding information, please forward the completed form to the Treasurer's Office.
(Note: MA State W/H Form only needed if different from Federal Form)
HIRD Form (Health Insurance Responsibility Disclosure form) This form MUST be completed by all benefit eligible employees not enrolled in the Town's health plan.
Waiver Form This form should be completed for all benefits the employee does not choose to enroll in.

Health Insurance
As a member of the Southeastern MA Health Group (SMHG), the Town of Norton offers Blue Cross Blue Shield of Massachusetts HMO and PPO health insurance plans.

Effective for plan year FY2014, the Affordable Care Act requires that employers provide a Summary of Coverage and Benefits (SBC) for each of it's plans, to all eligible employees. This information is available here on the website, or will be provided in writing upon request.

Dental Insurance
The Town offers 3 dental plans through DMS and Altus. The Dental Overview provides a description of each plan and includes the rates for FY 2013. Enrollment Form
Vision Care Plan
As of July 1, 2013, the Town will offer a vision care plan through Davis Vision. This is a network plan with a very broad network of providers. Enrollment Form
Cancer and Accident Insurance Plans
The Town offers both Cancer and Accident coverage through Allstate Insurance Company. These plans are administered by LifePlus Insurance Agency, Inc. in Marshfield, MA. To enroll or for claims assistance, contact LifePlus at (866) 511-9222.

Basic & Voluntary Life Insurance
The Town of Norton administers, through Boston Mutual Life Insurance Company, two group life insurance policies to it employees at the time of hire. They are the Basic Life Insurance Plan and the Voluntary Plan. The Basic plan provides a $4,000 Death Benefit and a $4,000 Accidental Death and Dismemberment Benefit. The Town pays 75% of the cost and the employee pays 25%. The Voluntary Plan provides coverage up to $400,000 and is available at the employee’s expense. Short Term & Long Term Disability Insurance is available through Fort Dearborn Life Insurance Company. (Rates and Enrollment Form)
Flexible Spending Accounts (FSA) are a way for employees to set aside pretax dollars to pay for out of pocket medical/dental expenses, prescription drug co-pays and dependent care costs. Norton employees have the option of establishing a FSA through Cafeteria Plan Advisors Inc. (CPA). The enrollment period for this benefit is the month of November for the following calendar year. As of January 1, 2013, all Medical Flexible Spending Accounts will include the "Benny" Card. This card is provided at no cost to the employee and is meant to be used like a credit card at the time of service instead of paying out of pocket and submitting receipts for reimbursement later on. Deferred Compensation Options
Employees may invest pretax dollars in a 457 plan through the Mass Smart Plan or Met Life.

School department employees also have the option of investing in a 403b account if they choose. The Town's 3rd party administrator for the plan is TSA Consulting.
